Remuneration: Circa £110,000+

Description: Full or Part time Performer Dentist required for 2-5 days (flexible) in a busy, mixed INDEPENDENTLY OWNED PRACTICE near Rotherham, South Yorkshire.
•             £14.50 per UDA
•             GOOD PRIVATE POTENTIAL
•             50% split on private & lab bills
•             2 Associates & 2 Therapists on-site
•             Fully computerised surgeries - 3 surgery practice
•             Digital x-rays
•             Rotary endo
•             Will accept newly qualified associates
•             Experienced Practice manager on site
•             Full support team
•             Existing well maintained list
•             CAR PARK ON SITE
•             15 minutes to Leeds
•             Available now

Compliance requirements:
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate

- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
