Job description
Be the first point of contact for any support requirements in relation to enhanced care delivery (1:1 or cohort nursing). Work collaboratively with others to identify and implement improved processes that meet audit standards and improve the temporary staffing service across the Trust. Plan and manage own clinical and managerial workload. Plan individual patient risk assessments and care plans to initiate and/or organise case conferences with mental health or ICB partners as clinically indicated. Provide professional support and expert clinical advice as required to the Trust Produce and present reports including information and data at internal and external meetings. As part of supervision, review process notes, care plans and risk assessment quality, addressing any actions required. Exercise expert clinical judgement when dealing with the holistic care of patients and carers, taking into account complex family aspects of their care. In collaboration with the executive lead for mental health, design and lead the development of the service provided to patients with mental ill health whilst in acute care. Develop an understanding of the mental health needs of those patients under the care of the Trust who have long term conditions, developing sign posting and pathways to ensure appropriate care is provided. Lead in operationalising strategic plans to develop the Enhanced Care Team. Develop and deliver project and action plans to support proposed changes ensuring that milestones and targets are met using QI methodology and approaches. Assist in the planning, delivery and evaluation of effective services within the locality which meet the needs of the service users and the expectations of commissioners and ICB. Work collaboratively and dynamically with partner organisations, on health and care partnership priorities, to improve the care provided to patients with mental ill health. Plan the implementation of policies and service developments within the team. Assist in the development of the workforce key performance indicators to monitor and measure the performance of the Enhanced Care Team. Provide strong clinical leadership, management and support on a day-to-day basis for team members or wider Trust colleagues, whilst ensuring access to appropriate professional managers over matters of professional expertise.
