Job description
To undertake the role of primary nurse for a designated group of service users, be responsible for the assessment of care needs, and the development, implementation and evaluation of programmes of care for that group of service users. To provide clinical leadership, supervising, supporting and appraising junior members of staff including healthcare support workers and students as required. To undertake the role of shift co-ordinator or security nurse within the unit as required. Ensuring the implementation of agreed care plans, high quality nursing care and evidence based practice within the clinical service area. To undertake the role of care co-ordinator to individual service user, providing, in conjunction with the MDT, specialist packages of care for service users with a history of offending behaviour linked to their mental disorder To facilitate comprehensive risk assessment and management plans using the HCR-20 and develop treatment and recovery plans for service users within a secure environment To maintain clear and accurate clinical/medication records whilst ensuring their practice complies with the Mental Health Act, Code of Practice, the NMC Professional Code of Conduct, and the policies and procedures of Leeds and York Partnership Foundation Trust (LYPFT)
