A Vacancy at Nottinghamshire Healthcare NHS Foundation Trust.

We are looking for a Highly Specialised Physiotherapist or Nurse to lead and manage the Pulmonary Rehab service in Mid Nottinghamshire; Mansfield, Ashfield, Newark and Sherwood localities.

Working collaboratively within an integrated multi-disciplinary team, you will provide specialist Physiotherapy/Nurse interventions and Pulmonary Rehabilitation to patients referred to the community Pulmonary Rehabilitation team.

The Pulmonary Rehabilitation team will work closely with other key organisations and provide on-going rehabilitation in a variety of community settings. The team will focus on timed specific patient goals and will embrace the needs and ability of this patient cohort

As a Highly Specialist Physiotherapist/Nurse for the community Pulmonary Rehabilitation service you will provide leadership and support to staff regarding the delivery of the specialist community Pulmonary Rehabilitation Programmes and ongoing case management as part of the specialist services. You will also have the following responsibilities:

Be responsible for the operational and clinical management of the team and service
This post will focus on clinical quality, service improvement, clinical governance & innovation, clinical incidents & risk management within the service.
Provide operational leadership; developing long term condition management within community services and ensuring commissioned targets & Key Performance Indicators are met.
As a specialist clinician, the post holder will provide highly expert assessment and treatment programs for complex patients who require Pulmonary Rehabilitation. Supporting staff within the team to manage complex patients who require Pulmonary Rehabilitation.
The post holder will collate patient data and contribute to national audit requirements such as National Asthma and Chronic Obstructive Pulmonary Disease (COPD) Audit Programme (NACAP)
The post holder will be accountable for ensuring the Pulmonary Rehabilitation service meets accreditation standards or working towards meeting standards of accreditation by the Royal College of Physicians (RCP) Pulmonary Rehabilitation Services Accreditation Scheme (PRSAS

The successful applicant will have the following qualification's, skills and experience:

HCPC/NMC Registration
Educated to BSc/Masters Level or equivalent in Physiotherapy/Nursing
Post graduate study in Pulmonary Rehabilitation
Experience to undertake advanced assessment, treatment, monitory and evaluation of highly complex specialist respiratory conditions
Experience to contribute to the strategic development of community based Pulmonary Rehabilitation pathways
Be a clinical educator
Experience of advanced clinical skills to undertake interventions consistent with evidence based practice in line with national and local policy
Continuing Profession Development and must demonstrate evidence in profile
Experience of leading in complex care coordination of specialism
Experience of proactively managing complex conditions, cognitive impairment, and mental wellbeing
Experience of supporting self-care, self-management to enable independence
Can demonstrate professional practice and leadership
Is a credible practitioner who can demonstrates sound professional, clinical and practical knowledge
A willingness to complete post registration courses in line with service needs and undertake a link role as required
Assist in the development, implementation and evaluation of educational programmes for primary care workers to provide the necessary knowledge and skills for effective management of long-term conditions
