Our professional responsibility is to reduce the risk of infection to others as far as possible. Getting vaccinated against diseases that can be passed person-to-person in healthcare settings is part of this responsibility. The Trust will provide advice, guidance and support on vaccinations and access to vaccinations where requested.
The post holder will be part of the Mother & Baby Unit Outreach Service across Somerset. The Outreach Practitioner will work with women who are admission vulnerable in the third trimester, and up to a year postnatally. This will involve delivering a robust care pathway for women in the community, seeing them through admission, during their inpatient stay & back into the community for up to four weeks post discharge, ensuring high quality co-ordinated care at all times.
They will ensure the delivery of these services in accordance with robust governance arrangements within the directorate which will include taking a lead clinical role in an area of service effectiveness. They will provide effective professional clinical leadership through appropriate modelling of behaviours and skills and/or direct supervision.
The role involves working across Somerset in the community, alongside Somerset Outreach colleagues, with some lone working. There will be some expectation, where appropriate, to travel to Jasmine Lodge to maintain contact with patients who may be admitted, or to attend MDT when this is not possible via Teams.
The role involves some home working and some provision of cover for the duty service on a rota basis, this operates 1000 - 1600 daily.
The working hours of the service are 0900 - 1700.
The Mother and Baby Unit (MBU) Outreach Service provides assessment and intensive support/treatment of women who would otherwise be admitted to our MBU - Jasmine Lodge, and facilitates early discharge from the MBU in order to prevent relapse and re-admission. The service predominantly supports mothers from across Cornwall, Devon and Somerset, although will accept admissions from other areas of the country if bed availability is limited.
Our MBU Outreach Service offers assessment and treatment of the mothers mental illness, whilst ensuring the developing relationship with their baby, focusing on bond and attachment. The service works with mums who are experiencing a severe episode of mental illness. This includes mums who might benefit from an admission to our Mother and Baby Unit (MBU) - Jasmine Lodge.
Jasmine Lodge is located in Exeter, and the practitioners from our Outreach Service are based across Devon, Cornwall and Somerset. They work collaboratively with other local health services and community perinatal teams to ensure mums and their families receive joined-up care, when and where they need it. Our service also includes access to a perinatal consultant psychiatrist, specialist doctor and clinical pharmacist.
This role is covering predominantly Somerset, working alongside the PMHT, however at times it may be necessary to provide cover across Devon.
This is a maternity cover post, covering one year.

Work closely with key internal and external personnel including midwifery, acute medical staffing, safeguarding, housing, voluntary and non-statutory agencies and a wider health and social Care community.
Ensure close liaison with primary health care staff and commissioning officers regarding referral criteria into the services and quality standards and ensure targets from the National Framework are met.
Liaise with professional stakeholder groups and interface service providers and agencies to develop mutually helpful working relationships.
Act as a point of contact for agencies outside Devon Partnership Trust seeking information about the team, or wanting to refer to it.
Present information, some of which may be contentious, to staff groups, service user groups, carers groups and other stakeholders using a range of techniques and media.
Investigate complaints, grievances and performance issues taking remedial action as necessary.

Actively participate in multi-agency forums to ensure the identification of need and the development and delivery of integrated local services in line with local need.
Ensure that activity is delivered in accordance with agreed quality standards and to service level agreements and in line with relevant national guidance.

Work closely with other teams with the Trust and the Voluntary, Statutory and independent sectors to ensure effective discharge planning which incorporates implementation of key success criteria for NICE guidance eg NICE 192 Antenatal Postnatal Mental Health, CPA, Information Sharing etc.
Implement a formal and active system of caseload review, caseload management and pro-active care management that will enable achievement of local and national targets.
Participate in safeguarding adults and children to level 4 as set out in safeguarding children and young people: roles and competencies for health care staff.
Ensure the development of local expertise, ongoing service improvement and professional monitoring of standards.
Undertake programmes of audit and research within the team and implement the findings.
Assist teams and individuals to negotiate changes in practice required to meet ongoing service developments.
Identify and implement best practice in the delivery of mental health services within the team and disseminate these throughout the service.
Participate in the maintenance of current services and future developments within the locality eg Care Pathways development.
Initiate service developments and projects. Plan provision of local services in conjunction with the service and other managers.
Participate in the service governance and research agenda within the locality eg Clinical Audits, NICE and Standards and Evaluation Research.
Ensure risk registers and Quality Improvement Plans are embedded within team structures and remain current.
Take responsibility for ensuring that multi-organisation governance is considered and managed appropriately.
Participate in the development, implementation and evaluation of multi-disciplinary client records.

Have an on-going awareness of high clinical risk situations within the relevant teams and advise clinicians on actions to be taken to respond to risk.
Attend other meetings relating to risk management where other agencies have a statutory role as chair.
Chair and participate in multi-agency Safeguarding meetings.
