Permanent / Contract: Permanent
Salary range: £Competitive plus Comprehensive Benefits Package.

Full time/part time: Full-Time, 37.5 hours per week, Shifts are Monday to Friday and are variable between 08.00 - 20.00.

We have a superb career opportunity for a Clinical Pharmacist to join this professional friendly team of pharmacists at the Cromwell Hospital. We have consistently innovated and improved how healthcare is delivered in our state-of-the-art private hospital, with cutting edge technology, expert medical professionals, and we always ensure safe patient centred care.

The Clinical Pharmacist is a key role in our pharmacy services. You'll work in collaboration with the wider pharmacy team and ward based MDT's to deliver and exceptional service. You'll be responsible, reliable, patient focused and knowledgeable, with demonstrable experience providing ward based pharmacy services.

You’ll help us make health happen by:

Providing a comprehensive and patient centred Clinical Pharmacy service to patients at the Cromwell Hospital including participation in the antibiotic stewardship round, MDT input, therapeutic drug monitoring, medicines reconciliation and dispensary support.

Facilitating and promoting rational and optimal medicines usage.

Providing assimilated and relevant medicines information to healthcare professionals and patients.

Working collaboratively with healthcare professionals to contribute to enhanced clinical pharmacy services.

All processes must be in line with RPS, GPh.C, MHRA, Home Office and BCH SOP standards.

What you'll do:

Contribute actively to service improvements on allocated nursing areas in conjunction with the multi-disciplinary team.

Carry out effective medicine reconciliation on admission and discharge.

Perform regular monitoring of medicines management on wards/departments including controlled drugs and advising nursing staff on legal control, safe custody and security of drugs.

Provide relevant medication counselling to patients to enable them to take their medicines safely and effectively.

Participate in ward rounds, clinical hand-overs and promote the role of the Pharmacist as an integral part of the hospital multi-disciplinary clinical team.

Ensure that the Inpatient Clinical Pharmacy service is patient focused, responsive and appropriate and that the professional image of the department is upheld at all times.

Key Skills / Qualifications needed for this role:

Member of General Pharmaceutical Council (essential)

Post Graduate Diploma in Clinical Pharmacy (essential)

Minimum of two years post-registration hospital experience.

Currently working as a Clinical Pharmacist on wards (essential)

Demonstrable experience in general clinical practice including medicine and surgery.
