Social Workers to work in a Hospital Discharge Team in a hospital in Portsmouth
To undertake Care Act assessments for adults who have been discharged from hospital under discharge to assess.
Hybrid role with an expectation of one or two days being based in Portsmouth.
Experience in undertaking CHC work would be an advantage.
3 - 6 months project.
