Swindon Borough Council has a unique opportunity for you to join the highly experienced, dynamic, progressive and expanding Reablement Team. We are a successful team who have been able to ensure that 80% of our service users have remained independent in their own home without any formal support. We are also part of a team who were highly commended by the Health Service Journal, for improving value in the care of frail older patients and we were part of a team highly commended for Integrated Discharge Planning and Reablement. Your next role
The Reablement Support Worker role is part of an integrated Reablement Team and the wider Adult Health & Social Care multidisciplinary teams, to provide support with personal care and reablement to service users in their own homes and community through outcome focussed support. This is achieved by supporting individuals to regain, improve and maintain skills of daily living, promoting maximum independence
