Job description

What you will be expected to deliver in the role To be responsible for managing own caseload of clients to include prioritisation of workload. To carry out person-centred assessments of the clients presentation based on a keen understanding of the young persons social, home and learning environments and determine appropriate course of action. Formulate and review individual care plans for each client using clinical reasoning skills and utilising a wide range of physical, sensory and cognitive treatment skills following analysis of information received from client, carers and referrer and any other professionals involved in the clients care as necessary. To attend educational, safeguarding children core-groups, Children in Need, Team around the Child and Looked After Children reviews as appropriate To provide specialist interventions and act as a resource to the wider team/service in area of specific expertise. To be responsible for implementing a range of interventions for individuals, families, carers, siblings and groups. To assist with providing information for Continuing Health Care Assessment for young people actively receiving behaviour intervention. To liaise with other professionals and the young person and their family to contribute to the completion of Health Action Plans, Life stories and This is me work. To provide behaviour support in a range of settings including home, school, respite and in the wider community. To develop individualised behavioural support programs appropriate to the complex holistic nature of needs of the individual and their families Facilitate groups in the delivery of health promotion activities and health education based upon evidence based practice Instruct clients and carers, providing direction and guidance on a range of behaviour management, communication and skills development activities, as directed as necessary by the named nurse, adapting to suit specific situational factors. To be responsible for implementing a range of interventions for individuals, families, carers, siblings and groups.
