As a Senior Nurse in our integrated mental health team you will be responsible for leading the perinatal mental health pathway. You will assess and manage a defined caseload of patients experiencing mental health problems and those with pre-existing mental health diagnoses within the perinatal period to aid reduction in risk and maintain stability in this vulnerable period for the patients mental health. You will work collaboratively with the multi-disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan. Offering support and advice to carers and relatives and to work in an integrated manner with key partner organisations and professionals. You will have line-manager responsibilities to a small number of staff within the team. Full or part time will be considered for this role.
