Job description
The Medway Low Intensity Support Team (LIST) is a small Locality based service, which provides ongoing non-intensive support for service-users who need to remain under the care of a mental health team but have achieved stability.
The LIST team aims to offer a patient centred service and involve carers and service users wherever possible within the framework of the team. The team will work in partnership with the carers support lead.
To support the Operational Manager within the multidisciplinary of the LIST team. Through professional, visible leadership that enables evidence-based practice; setting standards that result in high quality patient care and in line with KMPT Low Intensity Support Team Operational Policy.
To provide a medication service for those patients who are on medication which cannot be provided by their GP. Most GPs are currently unable to provide for giving depot medication and LIST has provision to include these patients. Depot medication is given at clinic, with some provision for home visits.
LIST will provide prescriptions for medication as required and will administer depot medication if this cannot be given by the GP or the depot clinics. LIST will arrange with the Trust’s pharmacy service to supply medication when appropriate.
