Job description


The post-holder will work closely with the practice manager, Clinical Lead GP and the Head of Nursing as the senior management team for the practice and will provide leadership at the practice for nursing and other staff. The post-holder will therefore have joint decision-making responsibilities with the Clinical GP Lead in relation to funding, clinical and management decisions and will (co-)chair the MDTs with the Clinical Lead GP. Working in accordance with the NMC Code of Conduct, the post-holder is responsible for ensuring the delivery of safe, effective and evidence based nursing care to the whole practice population. Working as part of a multidisciplinary team, they will lead and develop the nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.

1. The post-holder will have clinical and management/leadership responsibilities. Clinical work involves seeing patients presenting with undifferentiated / previously undiagnosed conditions as well as the ongoing care of patients.
2. Advanced Nurse Practitioners working at this level of competence must have the ability to use complex reasoning, critical thinking, reflection and analysis to inform their health assessments, clinical judgements and decisions. The post-holder is the senior nurse in the practice and works with the wider team and affiliated professionals to ensure that workload is effectively and appropriately shared.
3. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.

5. The lead nurse will work closely with her nursing team to ensure they are delivering the services to the needs of the community.
6. The lead nurse will work closely with the Head of Nursing to ensure all targets are met at the surgery.
7. Work closely with the Lead clinical GP and the practice manager to ensure there are appropriate clinics available for the patient cohort.
8. They will work closely with other Lead Nurses within the Directorate to ensure consistency in nursing practices.

The post-holder will develop practice-agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners. They will lead and manage nursing resources, working closely with the practice manager and GPs to deliver practice priorities. The post-holder will provide leadership and direction for the nursing team as well as advising the Clinical Lead GP and practice manager on health care priorities and service redesign. They will also have excellent organisational and planning skills in order to successfully lead the nursing team in the delivery of care and service. Additionally, the post-holder will have strong analytical, problem-solving and research skills in order to lead the nursing function and deliver the service to patients.

They will ensure that Quality Outcome Framework QOF indicators are being met and managed with the team
