Job description
We are recruiting a registered nurse to join our professional team of district nurses who care for people in their own homes. This includes care homes and occasionally nursing home input. We pride ourselves in caring for people at home and subsequently reducing hospital admissions and unnecessary disruption to patients and their families.
District nurses deliver many aspects of care to people in their own homes. This includes diabetic management, all aspects of wound care, palliative care, venepuncture, catheter management, holistic nursing assessment of needs, medication administration. All care is assessed using the Welsh Levels of Care to assess acuity. District Nurses work collaboratively with other MDT's and closely with GP Practices in the local area.
The team have a professional, evidence based work ethic and is led by a manager with a plethora of community experience. The team can offer a unique 'work family' environment and the opportunities for career development are available and varied.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
