Job description

To triage referrals within Urgent Community Response Team as part of the multidisciplinary team To assess and prescribe care pathways for patients in a crisis situation, with long term conditions, the frail & elderly, palliative and end of life care and/or rehabilitation needs, to achieve quality of life and independence where possible. To assess, prescribe and deliver care packages for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible. To work within the integrated team to prevent unnecessary admission to hospital via an unplanned care Home First Hub. Able to triage, review timely responses to referrals on a needs-based basis utilising the skills and expertise of others in the team. To work within the integrated team to support early discharge from hospital. To work within the integrated team to prevent unnecessary admission to hospital. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
