Description: Part or Full time Locum Dentist required for 2-5 days (flexible) in a busy, mixed, well established INDEPENDENTLY OWNED practice near Pontefract, West Yorkshire.

•             Long standing support team
•             Existing, well maintained list 
•             Separate decom room
•             IMOS contract
•             FD Training practice
•             Denplan and Diamond Invisalign Provider
•             State of the art air conditioned surgeries
•             Practice offers implants, oral surgery and ortho.
•             30 minutes to Doncaster and Wakefield, 40 minutes to Leeds, 45 mins to Sheffield. 
•             Available now, negotiable start date

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate

- Clinical references covering places of employment over past 2 years (minimum 2 referees)
