Community Diabetes Specialist Nurse

Band 7

37.5 Hours per week

An exciting opportunity has arisen for a Diabetes Specialist Nurse (DSN) to be at the forefront of driving up standards and quality in diabetes care within Liverpool Community Diabetes Services.

The role will involve working in a variety of community and primary care clinical areas, including General Practices and community clinics. This involves working with health care professionals across community and primary care services alongside the diverse communities of Liverpool. 

The successful applicant must have experience of working autonomously within the field of Diabetes, working in line with evidence-based guidelines to support clinicians to improve outcomes for people living with diabetes in Liverpool.

The post holder will be required to deliver complex educational packages to health care professionals, patients and other partners around effective diabetes management. The role includes autonomous clinical decision making and will be required to independently prescribe safely in a variety of community settings.

Main duties of the job

Act as Diabetes clinical expert and professional resource to Multi-Professional community teams, patients and carers within Liverpool.

The post holder will support the development of diabetes care in partnership with other members of the speciality teams across Acute, Community and Primary Care Service.  

The post holder will carry responsibility for a defined case load  and offer highly specialist advice, setting standards of care, the assessment of care needs and the development, implementation and evaluation of programmes of care, within the Primary Care Networks across Liverpool

The post holder will contribute to achieving the objectives of the clinical service group/speciality corporate objectives and work within the clinical governance framework

They will lead the development and delivery of a programme of practice and professional development for delivery of care within the specialist area across a variety of community based clinical settings
