The Mental Health Care and Support Team comprises social workers, occupational therapists and assistant practitioners and provides a social care service to adults whose daily lives are impacted by mental illness or impairment.The post-holder will undertake social care assessments and reviews under the Care Act 2014 and hold a caseload of service users requiring complex case management.
They will carry out direct work with service users to meet identified social care needs, carry out safeguarding investigations where required and develop strengths-based support plans to promote service users’ wellbeing and independence.
Job Purpose:
• To work as a social worker within the Mental Health Care and Support Team, supporting adults with mental health needs.
• The post-holder will undertake social care assessments and reviews under the Care Act 2014 and hold a caseload of service users requiring complex case management.
• Carry out direct work with service users to meet identified social care needs, carry out safeguarding investigations where required and develop strengths-based support plans to promote service users’ wellbeing and independence.
• The post-holder will need to be able to work creatively with service users who have complex needs or are hard to engage
