Remuneration: Circa £70,000-£80,000+

Description: Part-time Dentist required for 2-3 days per week in a busy, mixed INDEPENDENTLY OWNED PRACTICE near Leamington Spa. 
•    Open to a Locum if can commit for atleast a few months
•    3000-4500 UDAs per annum @ £13.50 per UDA
•    50% split on Private and Labs
•    9am to 5.30pm
•    High grossing private practice (70% private) with a lot of opportunity to deliver cosmetic treatments including ortho & bonding
•    Fully computerised surgeries - 4 surgery practice
•    Digital X-rays
•    Rotary endo
•    2 Hygienists/Therapists
•    Experienced nurse provided
•    FREE street parking
•    Existing and well-maintained patient list
•    Available now, start date negotiable

Compliance requirements:
- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number
