Job description

Communication Communicate in an efficient and, polite and courteous manner at all times. Communicate and delays, liaising with staff and patients. Effective use of patient information and other IT systems. Discussing with senior staff any resources required to provide service delivery Service Delivery and Improvement To be an effective member of a multidisciplinary team under the direction and supervision of a Registered Nurse (RN). To ensure that all patients are received and processed efficiently into clinics in a courteous and efficient manner. To arrange for the expedition of very urgent patients requiring outpatient appointments To book outpatient follow up appointments when required within the timescales required by the clinician ensuring that the Clinic capacity is strictly adhered to. To co-ordinate the input of demographic, personal and scheduling details of patients hospital attendances and outcomes using the patient information system (SEMA) to provide the Trust with an up to date record of patient activity and information. To ensure Secondary Outcomes are entered into the patient record to enable tracking through the health care pathway. To track in all patients case notes using the electronic Patient Document Tracking System to ensure that current location is recorded at all times on the system. To implement Trust policies and departmental procedures for own area and propose changes to administrative policies. Maintain a safe environment. Adherence to Trust Infection Control Policies. Be flexible and committed to working cross-site in all Gynaecology clinical areas People Management and Development Ability to use own initiative. Participate in the promotion of a positive learning. Act as a role model at all times Patient Care Delivery To actively take part in meeting the needs of the patients under the supervision of a qualified nurse, working within the Trust's policies and procedures and the philosophy of the department. Take and record patient observations. The post holder will have direct contact with patients in their capacity as an HCA within the unit, under the supervision of a RN. Where appropriate, to continue to develop own capabilities in nursing tasks in order to deliver a high standard of care under the supervision of a RN. To act as a patient advocate when chaperoning and provide support to women receiving difficult or bad news in clinics. To participate in ensuring effective use of resources and reduction of wastage within the department. Be committed to working as a Healthcare Assistant and Receptionist in order to meet the needs of the service. To participate in the reception of patients attending the unit to ensure that they experience a smooth and effective journey through the department, this will include working closely with all staff in this area. To handle all enquiries in an efficient, polite and courteous manner providing a caring and helpful service to NHS patients. To be the patient advocate including chaperone Provide emotional support particularly when patients are upset. Support a patients physical ability and perform personal hygiene. Perform observations and basic tests eg Blood Pressure, Pulse, Respiration, height, weight to calculate BMI, urinalysis and pregnancy tests. Learning and Development Attend mandatory training updates as required. Commit to completing the 2-week induction training and Care Certificate within the first 3 months of employment at the Trust. Undertake training as necessary in line with the development of the post and as agreed with the line manager as part of the personal development process. Achieve and demonstrate agreed standards of personal and professional development within agreed timescales. Identify own learning needs and jointly plan training requirements with your line manager Participate in the Trusts appraisal process to discuss how your role will help deliver the best possible care to our patients and help to deliver any changes in service This job description is an outline of the role and responsibilities.
