Job description
Are you an accomplished children's nurse with strong leadership skills and a passion for community nursing? We have a rare and exciting Job opening for a compassionate, dynamic, and inclusive Team Leader to join our CCN Team in West Dorset.
You will be responsible for the daily operational management and leadership of the team Monday - Friday 9.00am to 5.00pm providing direct nursing care, support and education to children and young people with acute nursing needs and those with long-term conditions / complex medical needs and their families. The team provides a service to children and young people aged 0 - 18 years and works across West Dorset.
Applicants must be NMC Registered Nurse (Child) with recent experience as a Band 6 Nurse. Evidence of a wide experience of children's nursing, including caseload management and of working within the community setting is essential.
To take continued responsibility for the assessment, management and evaluation of skilled nursing care in negotiation with the child and family To work in a collaborative manner with other agencies to ensure a seamless delivery of care to families To provide information and support to facilitate the childs and familys own choices with regards to nursing care To promote Health Education as an integrated aspect of care delivery To lead complex discharge planning and assessment of a childs needs, ensuring detailed communication with discharging hospital and safe discharge for the child and family To maintain accurate, systematic and timely record keeping including recording of clinical data.

To operationally manage, provide leadership, professional advice, and care for the Community Children’s Nursing Team in West Dorset. The post holder may have responsibility for management of a designated caseload delegating responsibilities and tasks as required.
Responsible for the nursing leadership and operational management of the Childrens Community Nursing Team. Ensuring visible leadership, maintaining high clinical standards, encouraging innovation, and providing child and family centred care.
The post holder is responsible for ensuring Trust Policies and national standards / programmes are implemented / met and that the voice of the child and family are central to care delivery.
To act as a lead resource to members of the wider clinical team including Primary Care Teams and other professionals advising on care, policy, and service development.
To establish, develop and lead a development programme for the nursing team and the service. To ensure that policies and guidelines are in place, with mapping of the caseload and accurate recording of activity.
To ensure the child remains the centre of all care and the child and family have a strong voice to guide care planning and make informed choices.
