Caseworker - Grade C
Description
We are seeking a dedicated and compassionate Caseworker to join our team. As a Caseworker, you will play a key role in providing support and assistance to individuals and families in need. Your primary responsibility will be to assess clients' needs, develop and implement care plans, and coordinate services to ensure their well-being and success.
To thrive in this role, you must have excellent communication and interpersonal skills, as you will be working closely with clients from diverse backgrounds and collaborating with other professionals and community organizations. Your ability to empathize, listen, and build trusting relationships will be crucial.
Responsibilities
Conduct comprehensive assessments of clients' needs and strengths
Develop and implement care plans tailored to clients' individual circumstances
Coordinate service delivery and liaise with other professionals, agencies, and community resources
Provide counseling, advocacy, and guidance to clients and their families
Monitor and evaluate clients' progress towards their goals, making adjustments as needed
Maintain accurate and up-to-date client records and documentation
Collaborate with multidisciplinary teams to ensure integrated and holistic care
Requirements
Bachelor's degree in social work, psychology, or a related field
Previous experience in social services or case management preferred
Strong knowledge of current social welfare policies and practices
Excellent communication and interpersonal skills
Empathy and ability to establish rapport with clients
Strong organizational and time management skills
Ability to work independently and as part of a team
Proficient in computer applications and case management software
Hours Per Week: 35.00
Start Time: 09:00
End Time: 17:00
Pay Per Hour: £11.18
Location: Plymouth, South West England
