Job description
Provide high quality effective patient care Investigate and refer patients as appropriate in line with current NICE guidance Carry out both face to face and telephone consultations; x15 during morning session and x18 during afternoon session Carry out baby checks weekly or as required Deal efficiently with results and correspondence Ensure appropriate clinical coding to support clinical audit and Quality & Outcome achievements Ensure accurate and robust note keeping Ensure excellent communication skills with patients, carers and their families Ensure attendance and contributions at weekly meetings and educational sessions Build strong working relationships with Partners and the wider practice team ensuring collaborative with all staff Effectively manage own time, workload and resources Ensure compliance with on line mandatory training Attend required training events e.g. PLT and feedback Proactively report and consider any Learning/Significant Events Liaise with team members and external organisations involved in patient care and respond promptly to queries Share concerns and gather clinical insight from peers where needed Lead areas are encouraged where special interests apply
