Job description

CLINICAL Assessment and Treatment Planning To work autonomously to undertake comprehensive standardised and non-standardised assessments of clients with a range of health and social care needs and multi-pathologies; using advanced clinical reasoning, analytical and communication skills to determine diagnosis and prognosis of condition. This will include an assessment of patients physical, cognitive, mental, social and environmental levels of functioning and will require a sound knowledge of normal movement and function. To assess the needs of an acutely unwell patient and select appropriate management plan including supporting of complex discharge for end of life care. To assess impact of presenting condition(/s) on all aspects of patients life (i.e. activities of daily living, community participation, vocation etc) and determine appropriate management plan including onward referral as appropriate. To participate in the assessment of capacity and gain informed consent for OT assessment, intervention and information sharing as indicated. This includes consideration of legal frameworks regarding working with patients/clients who lack the capacity to consent to treatment. To undertake specialist assessment of manual handling requirements and environmental considerations to prescribe appropriate specialist equipment to optimise independence and rehabilitation while ensuring safety of patient, staff, relatives and carers (including performance of risk assessments). To assess for, prescribe, and fabricate orthotic devices. This will include initial and ongoing training in safe management (staff, family and patient), evaluation of prescription and adjustments to meet the changing needs of patient/client. To assess for wheelchairs (including pressure relief requirements, accessories). To formulate, plan, deliver and adjust a client-centred assessment by the use of effective and efficient clinical reasoning, supported by evidence-based practice. To delegate appropriate tasks effectively to rehabilitation assistants/students. To demonstrate the ability to analyse and interpret clinical and non-clinical indicators and information using advanced clinical reasoning to form accurate diagnosis and prognosis in a wide-range of clinical conditions. This includes the ability to clearly inform the patient and/or family/carer of the assessment outcomes. To formulate collaborative treatment plans with patients/clients, carers and colleagues, with demonstration of expert clinical reasoning to determine agreed objectives across both health and social care. To have clinical autonomy to co-ordinate the management of a caseload of patients. This involves taking a lead role for allocated patients within the multi-professional team to plan, deliver and evaluate all aspects of the patient treatment process including referral onto specialist or community-based health or social care agencies. To monitor the responses of patients/clients during assessment and treatment and adapt techniques, and review goals/objectives as indicated. Interventions and Treatments To formulate client-centred programmes of OT interventions based on current evidence and a sound knowledge of available treatment options, utilizing highly specialist clinical assessment, reasoning and treatment skills. This may include gait rehabilitation, task-orientated practice, therapeutic handling techniques, patient and carer education, adjunctive therapies, cognitive retraining approaches and other options. To use different treatment modalities i.e. manual techniques (therapeutic handling, facilitation and positioning), skills development/rehabilitation and compensatory techniques or prescription of medical devices, task analysis, patient education and splinting. To be able to utilise highly developed therapeutic handling skills to support rehabilitation of neurological patients. To be able to create and implement rehabilitation programmes with patients, families and carers including education, compensatory and restorative strategies. To provide training, education and advice to patients and carers on diagnosis, rehabilitation potential and management strategies and assist patients/carers to implement effective physical, psychological or cognitive coping strategies. To empower and encourage patients and carers where appropriate, to actively participate in and contribute to their rehabilitation. To identify and manage all risks associated with own clinical and professional practice and ensure all staff do likewise. To support the multi-professional team to ensure the safe and efficient discharge of patients from hospital including liaison and negotiation with a range of service providers, assessment of clients home environment, including social supports and house design/layout. To initiate and undertake risk assessments when assessing and treating patients. This will involve assessment of the individual and the environment and include advising the multi-professional team on appropriate management specifically relating to moving and handling and safe use of medical devices. To represent OT and/or individual patients at the multi-disciplinary team meetings to ensure the delivery of co-ordinated multi-professional care, integrating occupational therapy management into the treatment programme. This will include discussion of patient care, progress and potential. To evaluate patient progress, reassess and update treatment programmes as required through the monitoring of clients medical and rehabilitation needs. This will involve determining/diagnosing the patients potential for rehabilitation and future care and support needs. To understand and apply National Guidelines and legislation relating to health and social care in occupational therapy practice and support other staff to do likewise. Ability to participate and perform OT assessment, interventions and discharge duties as part of the weekend roster.
