Primary Care Clinician - Kirklees Care Home Support team

Your new role

We are looking for a compassionate and experienced healthcare professional (nurse, paramedic, pharmacist, AHP) whose job would be to assess residents in care homes across Kirklees and Calderdale. The service is provided by an NHS-funded community health provider. A typical shift will involve visiting 1-2 care homes per day, and assessing patients with long term health conditions such as asthma, COPD, diabetes, heart failure and heart disease. You will also support the on-site management and care teams to provide the best possible care for residents.

• Suitable for nurses, paramedics, pharmacists, AHPs with primary care experience
• Visiting care homes to assess residents who have long-term health conditions
• Flexible work pattern, full time or part-time hours available
• Contract until at least the end of March
• Service is operational Mon-Fri, 0900-1700

What you'll need to succeed

• Valid registration with professional body (e.g. NMC or HCPC)
• This role would be suitable for community and practice nurses, or paramedics and other AHPs with primary care experience
• Minimum of 1 years' experience in a similar role.
• Available to work at least 3 shifts per week between Mon-Fri
• Knowledge of long-term conditions and their management.
