Permanent, Full time (37.5 hpw), some occasional weekend work may be required

As our Lead Support Worker, you’ll be at the heart of supporting our customers who have complex mental health needs to develop their skills to live more independently in the community. Typical day as a Lead Support Worker (known by us as Coordinator)
- Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
- Our customers live across Darlington in one of our dispersed supported properties or in their own homes. We currently have 9 properties, and we are continuously growing our service so that we have 15 properties. We deliver support to customers who live in their own homes as well and we are looking to continue to grow that part of the service. Each customer has their own bespoke support plan, and each package of support will be different to the next.
- Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
- Carrying out risk assessments, support planning, goal setting and regular reviews.
- Following referral completing thorough assessments to understand if customers are suitable for the DD- STEP service.
- Working in a multi-agency approach to ensure our customers receive the support they need to successfully live in the community.

You bring
- Passion to support our customers to live their best life, working collaboratively with an eye for detail.
- Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
- Experience of coordinating and assessing customer referrals.
- The ability to work on your own initiative, remain calm under pressure and have a resilient approach.
- To get from A to B, you’ll need a vehicle insured for business purposes. The great news is that we’ll pay your mileage!

Job details
- Where we’re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
- You'll work Mon- Fri 9am- 5pm.
- Although you’ll work from home, the office or lone work, we do get together regularly for training and team meetings.
- Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
- You’ll need an Enhanced DBS check done and we pay for that.
