Job description
KEY DUTIES AND RESPONSIBILITIES: Work in partnership with other health professionals and external agencies to provide a personalised service to meet individuals needs. Undertake first visits to meet CQC compliance on behalf of the Home First service as requested by the Shift Leads. Read, follow and implement rehabilitation plans in accordance to the patients goals clearly documenting their progression. To monitor and report any changes to the patients individual plan of care that contributes to a continuing review of the patients care. To work unsupervised in patients homes in the community. Promote the patients privacy and dignity whilst undertaking rehabilitation using all personal activities of daily living for example with personal care such as; bathing, showering, toileting, hygiene continence care, skin integrity, meal prep and medication. Identify and escalate any reduced patients allocated time or rehabilitation visits as evidence supports and communicate changes to management or health and social care professionals. Maintaining the patients personal information and confidentiality at all times in line with MPFTs policies. Ensure the patient is involved in all decision making regarding their rehabilitation plan working with all health and social care professionals. Promoting independence with medication, compliance and concordance enabling self-management where possible in line with Trust Home First Medication Policy. Support and educate patients in managing long term conditions to maximise their independence which will includes following rehabilitation goals set by a competent health and social care worker. Promote and educate patients in managing their own personal safety for example referral to the fire service, ensuring that the property is secure or educating patients on potential environmental hazards that could cause a fall. Support patients with palliative care needs. Where the patients requires support in their decision making escalate appropriately to management or other health and social care professionals. Provide education and information on other internal / external services that may be beneficial in meeting the patients long term needs. Monitor and record patients health and wellbeing including observations and act on or escalate any concerns or changes in the patients condition, needs, or circumstances as appropriate Support patients with accessing assistive technology if appropriate to promote independent living. Work in partnership with the Therapy Teams to promote exercise programmes in accordance to the patients individual rehabilitation plan. Comply with all policies, procedures and legislative frameworks. The post holder is required to contribute to the achievement of the service Ensure a work environment that protects and promotes peoples health, safety and welfare. To report and escalate in accordance with Trust Policies which includes any safeguarding concerns and promote the welfare of vulnerable adults. Participation and contribution in individual and peer supervision sessions, scheduled team meetings and any personal development conversations. Ability to support with clinical tasks according to appropriate procedure and protocol following training and completed competencies. Demonstrate an understanding and compliance to meet the Care Quality Commissioning regulations.
