Pay Rate: £45.76 per hour PAYE
Hours per week: 40 Monday – Friday, normal working hours

Leading the strategic development of Brent Integrated Care Partnership’s health inequalities programme to address health inequalities; working in partnership across the Council, the NHS, including primary care, the voluntary and community sector and local communities;
Take the lead for key partnership agendas, boards and projects working with internal and external key partners including customers, carers,voluntary and community sector and statutory partners to build strong relationships which contribute to the delivery of the Council and Brent ICP objectives, and ensure the reputation of the Council and Brent ICP both within Brent and within NWL.
Work with NWL Integarted Care System in order to identify and realise opportunities to progress action to address health inequalities within Brent
Key Responsibilities
- Work collaboratively with colleagues within the integrated and partnership team across council, and make a proactive contribution delivering corporate objectives to tackle Health Inequalities
- Lead and manage a portfolio of customer focused services in alignment with both corporate and departmental aims and priorities.
- Provide leadership and management to achieve high performance and effective operational delivery. This will include managing the effective use of resources and staff, and providing leadership, expert guidance and support to the team managers and staff teams within your service area.
- Work closely with the ICP Managing Director to support effective working relationships with relevant portfolio holders in the council, and Voluntary sector
- Support and develop partnership working, including acting as an effective ambassador and advocate with external organisations and across the wider Council. This will include acting as the departmental lead for key partnerships and board, such as Health Inequalities and Vaccination Executive.
- Direct line management responsibility for the Brent Health Matters team
- Operational management of Brent Health Matters, reporting into the Health Inequalities Executive of the Brent Integrated Care Partnership; working with local stakeholders and communities to identify key areas of concern, developing and implementing strategies to address inequalities montoring progress and outcomes
- Lead and model a culture of collaborative working, focussed on holistic services that deliver good outcomes for the people of Brent, developing and modelling new ways of working including new models of co-production, contributing to the development of the evidence base on addressing health inequalities and ensuring the impact of this work is evidenced.
- As appropriate, and particularly where cross organisational leadership and facilitation is required, take on leadership roles which across the Brent Integrated care partnership (ICP), such as the Better Care Fund
- Responsible for the overview working of Brent Health Matters Team, ensuring proactive engagement with communities, primary care, ensuring the team is delivering on key priorities in tackling Health Inequalities across Brent.
