Salary £40,000 - £45,000 dependant on experience.

We are seeking a compassionate, enthusiastic and dedicated individual to join our friendly, award-winning team as Specialist Memory Care Lead.

ABOUT THE ROLE

Your focus as Specialist Memory Care Lead will be to support the team which works across the organisation. The role is predominantly based in the North and Central divisions of the Organisation and will involve regular travel and nights away from home.

As a Specialist Memory Care Lead you will support Avery’s Head of Memory Care to e nsure that evidence based best practice in person-centred dementia care is embedded across the organisation in line with all aspects of the Avery Dementia Strategy.

Other responsibilities include:

• To facilitate a variety of dementia training modules to staff in Avery homes as part of the Dementia training pathway.
• To work with and coach Memory Care champions within our homes to ensure teams have access to best practice resources.
• To work alongside staff in Avery homes to achieve identified outcomes in relation to individual residents or as part of wider planned developmental work.
• To support staff working in Avery homes with dementia specific care planning, including to support residents experiencing emotional distress.
• To promote positive risk taking for residents with dementia within a sensitive and robust risk assessment process that focuses on ability rather than disability.
• To provide education and support to the families of residents with dementia living in our Care Homes.
• To complete Home Visit Reports in a timely and supportive way to contribute to the developmental action plan for individual homes.

About You

Our ideal candidate must:

• Hold a valid UK Drivers License and own vehicle.
• 3 years’ experience of working with people with dementia.
• Hold a recognised health and social care qualifications.
• Flexible with regular over nights stays
