Pharmacist Pay Rate: £35 - £36 per hour (assignment rate)
Location: Birmingham
Contract Type: Locum – 3 Months

Main Duties:
- Develop a systematic medication review format to undertake medicines use reviews and medication reviews.
- Carry out clinical, level 3 medication review for patients in intermediate care units and for other patients as requested by the Case Managers. Assist staff in identifying those patients who would benefit from pharmaceutical intervention. This includes reviewing drug chart for correct dosages, drug interactions, side effects, legality and clarification of ambiguous prescriptions. Cancelling medication when the course is complete.
- Produce medication review reports to promote high quality, cost-effective prescribing which address prescribing objectives in line with the BCHC Formulary. Discuss the recommendations with prescribers and implement agreed changes
- Provide medicines information/counselling to patients and training to carers and care staff around medicines related issues and contribute to support concordance.
- Record medication review changes and audit as required to demonstrate the value of interventions made. Keep records of reviews and issues arising to identify trends. Feedback identified prescribing issues to prescribers. Identify key issues for community pharmacists to address during Medicines Usage Reviews to maximise their input.
- Subject to being qualified, undertake Non-Medical Prescribing for clients within competencies as agreed with the Chief Pharmacist.
- Assist with / identify and develop audits, where appropriate
- Liaise with community pharmacists, community nurses, hospital pharmacists other healthcare staff and social services as required to ensure medication changes are implemented smoothly. Work with appropriate team members to facilitate seamless admission and discharge in respect of medicines – medicines reconciliation.
- Support the palliative care and end of life care medication requirements for people in the intermediate care units.
- Provide pharmaceutical input to the Expert Patient and disease specific programmes developing material / presentations, as required.
- Provide medicines information (including therapeutic drug monitoring), pharmaceutical advice and support to the Intermediate Care Teams, Rapid Response Service, Integrated Multidisciplinary Team and Complex Care. This includes support with medicines practice and procedures. Provision of medicines advice in specialised areas especially with regard to long-term conditions and using medicines outside their product licence and dose guidance in the elderly and infirm. Liaise with Intermediate Care Unit managers about Pharmacy issues within the unit. Advise managers of legal and statutory requirements around medication.
- Address issues identified from reports / documents issued by the Department of Health and National Bodies relating to both people with long term conditions and medicines interface issues, as agreed. This may involve liaising with other pharmacist and healthcare staff.
Requirements of the Pharmacist:
- GPHC Registration
- Previous experience within a UK based NHS Pharmacy setting
- NVQ or equivalent – Pharmacist qualification
