Your new role!

We are looking for an enthusiastic and dynamic Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist to work alongside our Hospice Outreach Team to coordinate, lead and manage the daily activities and services for individuals’ and those who care for them using our services. This post will provide a crucial role in supporting staff and driving forward our model of patient centered care to ensure we have the right people, providing the right care at the right place and the right time’.

The Hospice Outreach Team are the initial point of contact for all new community referrals and all telephone calls coming into the service.It acts as a front line to support patient’s and those who care for themin their own homes’, this includes care homes and nursing homes. You will be part of a rota of clinicians covering our Hospice Outreach Service 7 days a week, offering remote support (telephone support and video conferencing) as well as clinic and home assessments.

To succeed in this role

You will have proven community or in-patient palliative care experience at Band 6 or 7 or have equivalent level of experience of working in the following related areas: Paramedic, General Medicine, Neurology, A+E, Frailty, Respiratory Care or Oncology.

We are looking for a motivated clinician with the ability to work as an independent practitioner whilst being part of a responsive and resilient team. You will bring with you a wide range of experience in mentoring and supporting individuals and teams and possess advanced communication skills and excellent presentation skills (both oral and written).
