Job description
The objectives of the service include: Reduced psychological distress and functional impact of health condition and/or treatment Improved treatment adherence and adjustment to health condition Improved individual and/or family function in relation to wider impact of health condition and/or treatment Improvements in psychological wellbeing and combined health/mental health outcomes Comprehensive Psychological Assessment Collaborative Psychological Formulations to inform understanding and intervention Identification and delivery of appropriate Intervention model and patient-identified goals Provision of psychologically informed resources/information and wider service signposting Referral on to partner agencies where required/necessary e.g. Child & Adolescent Mental Health Services; ASC (Autism) Pathway
