Exciting Opportunity in the Public Sector: Care Manager Adult
Our client in the public sector in Peterborough area is seeking a dedicated and experienced Care Manager Adult to join their team. This vital role serves as the cornerstone of support for both clients and support workers, ensuring the highest standards of health and wellbeing management within the service.
Key Role Objectives:
Serve as the primary point of contact for support workers regarding client incidents and health concerns.
Oversee medication support requirements, conduct audits, and address incidents and concerns efficiently.
Assist the Team Manager in ensuring service compliance with all CQC regulations and manage challenging client situations.
Provide managerial support and coordination within the service, acting as the first contact for the Reablement Coordinator.
Main Duties and Responsibilities:
Collaborate with multi-disciplinary teams to facilitate the reablement support process, ensuring effective communication and service continuity.
Manage relief cover, staff absences, and performance issues in line with policies and procedures.
Make critical decisions on matters needing escalation and ensure consistent delivery of medication support assessments.
Liaise with team managers and locality teams for operational continuity and adherence to health and safety procedures.
Maintain client confidentiality and contribute to the development of the Reablement Service.
Support performance management and engage in professional development.
