Remuneration: Circa  £150,000

Description: 

Part-time or Full-time Associate Dentist required for 3-5 days per week (FLEXIBLE WITH DAYS) in an INDEPENDENTLY OWNED PRACTICE in Ipswich.
•             £14 PER UDA
•             EXCELLENT PRIVATE INCOME - £12K-£14K PER MONTH
•             List of FPI, PracticePlan & NHS
•             Mentoring in implants available
•             4 surgery practice
•             8 Associates, Therapist & PM on-site
•             CBCT Scanner
•             Intraoral Scanners
•             Rotary Endo
•             Digital x-rays
•             Established, well maintained patient list
•             Experienced nursing team
•             Free on-street parking

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
