Job description
The post holder will be required to work with the Clinical Review Team to support with Section 117 Aftercare in Mental Health services. The Clinical Review Team liaises with all services and agencies to ensure that patient aftercare services are timely and any obstacles to this are resolved.
The post holder will have delegated responsibility for some aspects of the delivery of the service, under the supervision of qualified practitioners.
The post holder will arrange the Review meetings , minute taking and maintaining the spreadsheets to ensure accurate information is recorded. They will adhere to all relevant trust polices will and work as part of a team managing their own time and ensuring their workload is organised; balancing competing and conflicting priorities.
The post holder may be required to liaise with various teams, agencies, patients and carers under the supervision of a qualified practitioner. Excellent communication skills; both verbal and written are essential for this role.
Provide an administrative service including word processing, drafting, and producing for example, meeting notes, reports, and letters and business documents
Undertaking general office duties as required including filing, collating and distributing information, receipt and distribution of mail
To operate and maintain electronic systems as required
Maintain all spread sheets and databases including the production of regular information reports
Be responsible for organising and servicing team meetings/events as required, including room bookings, note taking, transcription and circulation
Procurement of stationary, staff uniforms and equipment stock
Use own initiative to meet tight deadlines
Provide flexible administrative and clinical cover as required
To work flexibly to meet the needs of the service.
To feedback (clinical information) all gathered information to the team and to undertake clinical support activity as requested
To maintain contemporaneous clinical documentation utilising electronic and other means with appropriate supervisions and support
To liaise with other professionals external to the team as appropriate
The job involves taking part in (clinical information) MDT’s and in the formulation of outcome plans devised in collaboration with the service user and professionals and carers

Support the process of service users as directed by the designated lead clinician
Work within their own level of competence to support the To implement specific aspects of the (clinical episode)
Promote and support the physical health and wellbeing of individuals in a holistic way utilising the principles of Making Every Contact Count
(Help) support individuals and services (services users, their family, their significant other) with their requirements for information
Provide advice and information as appropriate to enable the individual to manage their condition by adopting a suitable lifestyle to optimise their health and wellbeing
Work with service providers to enable them to support services users to access and use services in a way which respects their values and supports their rights
Once a decision has been made by an appropriate practitioner, support the transfer/discharge of individuals from or between health care services, in line with the relevant team and organisational policies
Support the effectiveness of therapeutic interventions in relation to supporting the recovery and wellbeing of service users
Be required to support the safeguarding of individuals.
