Job description
The post holder is accountable for supporting the Unit Manager in a Tier 4 specialised service for young people aged 12-18 years with an LD in the provision of care over a twenty-four hour period; they will work as part of a multi-disciplinary team in the planning, implementing and evaluating of comprehensive packages of care from admission through to discharge within a therapeutic environment. At all times acknowledging and respecting the individual rights of people with learning disabilities and / or mental health difficulties.
To assist in the management of the care team comprising of Qualified Nurses, Assistant Practitioners, Health Care Assistants and Support staff.
To provide comprehensive treatment packages from admission through to discharge for identified Service Users.
To deputise for the Unit Manager in all aspects of the job role during their absence.
To contribute to the provision of an on call / bleep holder rota for the Brooklands site.
Ensure that dignity, choice, respect, independence and the rights of the people with learning disabilities and / or mental health difficulties are
maintained at all times
Ensure the safety of service users at all times whilst at the same time promoting positive risk taking
Ensure service users are central to and involved in all aspects of their care.
Monitor and review care packages and make appropriate changes
Manage staff in line with Trust polices and procedures
Support the nurse in charge in the delegation of duties
Ensure continued personal development of self and staff team.
Work in partnership with other agencies and organisations
Have an awareness of national and local policy and the implications for service delivery
Participate in the monitoring and maintenance of standards in accordance with Trust and Unit polices and procedures, and Nursing professional standards
