Job description
Daily Task;
- Assisting customers with everyday tasks to foster their independence and support them in achieving the objectives and desired outcomes outlined in their individual support plans.
- Engaging with and supporting vulnerable young individuals
- Aiding customers with day-to-day activities, including meal preparation, shopping, budget management, maintaining their housing, and transitioning to independent living when possible.
- Conducting risk assessments, creating support plans, establishing goals, and performing regular progress reviews.
- Building relationships through meaningful conversations and regular interactions.
