To provide a direct service to children, young people and their families/carers, in line with relevant legislation, guidance, regulations and national standards.

MAIN DUTIES AND RESPONSIBILITIES

The job description refers to the principal duties and responsibilities of the Post. It does not necessarily list in detail all the tasks required to carry out these duties and responsibilities.

Duties

  1. To facilitate initial contacts with members of the public and signpost them to appropriate services, or conduct initial assessments to progress to referrals.
  2. To commission packages of care with support, via the service request form process.
  3. To undertake reviews/assessments of people with physical/sensory/age related impairment and the carers in accordance with statutory duties and relevant legislation, within an appropriate level of delegated responsibility.
  4. To arrange services, appropriate to meet needs, identified following assessment of service users.
  5. To have care management responsibility for a selected caseload.
  6. To accept referrals and assist qualified members of the Team and managers in particular areas of work including the monitoring and review of care plans.
  7. Maintain up to date case records and relevant documentation for Care Management.
  8. To input data and use the Department’s computer client record system to contribute to the maintenance and accuracy of the information.
  9. To undertake appropriate and relevant administration tasks including the collection and compilation of regular and ad hock statistical data as may be required.
  10. To attend and participate in Team and other meetings as required.
  11. To prepare for and attend supervision/appraisal sessions in accordance with the Community Services Supervision Policy.
  12. To identify, through supervision and in discussion with line manager, personal training and development needs and to attend training sessions as required.
  13. To be aware of the Department’s policies and procedures and relevant legislation necessary to effectively carry out the duties of the post.
  14. To carry out all tasks with due regard for the Department’s Equal Opportunity Policy and to promote the principles on non-discriminatory practice, service-user empowerment and the Social Model of disability.
  15. Ensure compliance with the Council’s Financial Regulations, Standing Orders, Policies and Procedures.
  16. To implement the principles of the Council’s Equal Opportunity Policy whilst carrying out the above duties.
  17. To adhere to Health and Safety legislation / relevant Council policies and procedures and to take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts / omissions.
  18. To implement the principles of the Council’s Environment Policy while carrying out the above duties.
  19. Such other duties and responsibilities commensurate with the grade and in accordance with the general character of the job as may reasonably be required by the Chief Officer from time to time.
