Detailed job description and responsibilities Communication and Working Relationships To provide clinical assessment and treatment for patients identified as having therapy needs, within Medical/Surgical/Emergency Services To explore innovative new ways of delivering acute patient services to facilitate speedier discharges from Hospital and participate in implementing and evaluating changes to the service To develop links with community-based services andIntermediate Care teams to ensure a continuous intervention To ensure holistic patient assessment including the physical, emotional and psychosocial needs of the individual To plan, implement and evaluate treatment programs for patients with a wide variety of medical conditions in liaison with multi-disciplinary teams, families, carers and other agencies To assess and make recommendations regarding care packages, equipment provision and ongoing treatment of patients with multiple needs to prevent hospital admission or facilitate discharge Maintain a high standard of documentation, records and audit information as required for clinical governance purposes To work without direct supervision ina variety of situations e.g. at weekends, bank holidays or on home assessment visits To complete specialised assessments to include cognitive, sensory and perceptual assessments wheelchair prescription and pressure-relieving equipment To manage own clinical caseload and other competing demands of job role, prioritising your own work and that of those you supervise To ensure that the framework for clinical supervision as expressed in the Trust Policy is embedded and supported through the clinical area in which you work In addition, the member of staff has a responsibility to access supervision at an appropriate level for their own practice To lead Case Conferences where necessary, ensure effective co-ordinated discharge planning To provide supervision of junior staff/support staff when appropriate including yearly appraisal and ensuring their continuedprofessional development To participate in own supervision and ensure own CPD To attend relevant case conferences, ward/clinical governance meetingsand servicedevelopment forums as requested by the team lead Participate in the recruitment and selection of staff as required by the ClinicalTeam Lead Occupational Therapist To be familiar with and adhere to Trust Health and Safety Policies and procedures, contributing to a safe working environment for staff and patients To contribute to service wide developments as directed by the Clinical Team Lead Occupational Therapist To participate in provision of in-service training for staff as required To ensure a high level of clinical acknowledge in the treatment of all medical conditions To undertake the education and supervision of Occupational Therapy students on clinical practice placements as required Participate in clinical audit/research as directed by Line Manager Participate in pilot/project schemes as directed by the Clinical Team Lead To maintain accurate up-to-date records including computer generated statistical information to produce patient related reports Ensure effective communication with all members of the multi-disciplinary team, Consultants, Ward Managers, Nursing Staff, Speech and Language Therapists, Physiotherapists and community-based colleagues in Health and Social Carer and voluntary agencies To liaise closely with the bed management team to facilitate effective use of hospital beds Communicate sensitive information to patients relatives and carers regarding the implementation of individualised treatment programs, loss of independence and unfavourable outcomes of assessments e.g. the inability to return home from hospital. Negotiate unwelcome changes in lifestyle with patients and carers Choosing appropriate communication strategies in order to communicate with patients suffering with conditions involving barriers to understand e.g. patients suffering with stroke, dementia and cognitive impairments PERSON SPECIFICATION Attributes Essential Desirable Qualifications and training Diploma/Degree in Occupational Therapy Valid state registration with Health Professions Council CPD portfolio and evidence of recent post-graduate learning relevant to area of clinical practice Have a minimum of 18 months experience within the NHS including experience working in a variety of roles both in the acute and community settings. Knowledge and experience Pressure care and prescription Elderly care rehabilitation skills General physical rehabilitation skills to include neurology, cardiology, respiratory and other medical conditions Ability to prioritise complex caseloads with patients with multiple problems Assessment for specialist equipment and minor adaptations Therapeutic and manual handling skills Detailed understanding of roles of multidisciplinary team members/health and social care agencies Knowledge of National Service Frameworks Knowledge of Government legislation and directives affecting health and social care Understanding of clinical governance issues Evidence of continuing professional development Ability to make fast accurate clinical judgement/decisions Ability to work autonomously and take a leadership role Experience of working in the Acute setting within the NHS Experience of Service Development Clinical leadership skills Student placement educator course Recruitment and selection training Experience in social care Wheelchair Accreditation Specific Skills Experience in a health/social care setting Experience working with Parkinsons disease, multiple sclerosis and other general neurological conditions Experience of working with palliative care patients Experience of working with falls and falls management Experience of working with complex and chronic conditions Use of specialist perceptual, cognitive sensory assessments and treatment techniques Ability to work independently as a lone practitioner without direct supervision Experience of student supervision I.T skills Experience of working with alcohol/drug abuse Experience of crisis intervention / fast track assessments Experience of working across service boundaries, with Social Services, Intermediate Care other agencies Experience of staff appraisal Experience of staff recruitment and selection Experience of service developments e.g. documentation, local standards of practice, protocols and service changes Experience of working in community hospitals Requirements due to work environment/conditions Experience of supervision of junior or support staff To undertake home/environmental visits Clinical Educators Course Car Driver Physical skills There is a requirement to complete Moving and Handling Assessments with patients and complete daily moving and handling practice as part of inpatient rehab. You will need to be able to use moving and handling equipment and transport equipment where and when required Physical effort The hospital setting is a busy environment with a high case load Emotional effort At times you will be working with patients who do not have good outcomes which can be emotionally upsetting. You may be required to deliver bad news or deal with conflict at times, which can be emotionally demanding Mental effort Carrying a high and often complex caseload will require a lot of mental effort
