Job description


Providing clinical expertise, education & support for patients approaching End stage Renal disease (ESRD) to enable informed decision making when choosing a treatment option as their kidneys fail.
To work in partnership with the Advanced Kidney care Matron & team, Frailty & AKCC Consultant and allied healthcare professionals in identifying patients who may benefit from the support offered.
To provide educational support to ensure that all patients have a frailty score when approaching ESRD in order to identify the patients who will benefit most from this service.

To work in partnership with primary care (palliative, General practice) to ensure good links that enable smooth referral where patients choose a supportive care pathway.

To work in partnership with the Trust palliative care teams.

To collect relevant data agreed with the Medical team & Programme Lead to demonstrate the outcomes of the intervention and provide monthly updates to the Divisional Tri that can be communicated to the ICB & NHSE.
• Clinical leadership & expertise in the specialty of Renal and/or advanced/supportive decision making & planning.
• Work autonomously, demonstrating innovation & resourcefulness.
• Provide a nurse led education & decision making clinic consultation service in partnership with the Consultant Nephrologist & Geriatrician to support patient decision making when approaching end stage renal disease..
• Assessment of patients’ individual needs and formulating / developing a plan of care. Monitor & evaluate this plan of care where patient has a change in decision making.
• Offer skilled emotional and psychological support and refer on to other members of the multidisciplinary team as appropriate.
• Active participation in multi-disciplinary meetings and other care providers if appropriate.
• Represents patients at multi-disciplinary meetings ensuring that their needs are assertively presented.
• Takes an active role in maintaining effective communication with all disciplines, patients and relatives.
• Ensures that all clinical and legal nursing documents are completed accurately and legibly and that their relevance is understood and confidentiality maintained in accordance with Trust policy and NMC guidelines.

• Assists with the coordination and manages complex case meetings with primary care representatives, social services and members of the multidisciplinary team.
• Participates in specialist studies and reviews that are undertaken Trust-wide and reflects a corporate approach in the implementation of practice.
• Provides a source of current highly specialist nursing information and teaches nurses and other disciplines.
• Partake in Renal Senior Nurses on call
