Your new company
We are currently looking to recruit a Qualified Social Worker for Bridgend Council\'s Hospital Discharge Team. As part of the hospital discharge team, you will be supporting individuals and their families or carers to adjust to illness, access support and plan for any post-discharge resources they might need. Working for Bridgend Council, you will be a part of a fast-paced, high-performing team that strives to provide the best possible care to all service users.

Your new role
You will have the chance to work on a hybrid working model, being able to work from home 2 days per week and be in the office 3 days per week. It is a full-time role of 37 hours per week and the rate starts from £35 per hour (umbrella).

What you\'ll need to succeed
- Social Work Degree (or equivalent)
- Full UK driving licence
- Registered with SCW
- At least 3 years\' experience
