Job description

The post holder will be expected to work as part of the integrated specialist diabetes team delivering diabetes care to adults in both in-patient and community settings Work as part of the MDT in specialist clinics such as ante-natal, pump, young adults and inpatient diabetes cover. Provide specialist expertise, knowledge, advice and education demonstrating a sound understanding of the management of patients in this area Demonstrate a high level of judgement, discretion and decision making in diabetes management within the acute or community setting, with an aim to reduce hospital attendance/admission and reduce length of stay during in-patient care Deliver education including structured programmes to people with diabetes to enable them to self-manage their condition Develop and deliver educational/training programmes and act as a specialist resource for staff and colleagues to improve standards of diabetes care Develop and implement diabetes policies and standards based on national guidelines, as well as provision of specialist care Monitor and improve standards of care through supervision of practice, clinical audit, evidence based practice, learning and development and the provision of skilled clinical leadership Support the service lead with service improvement development The Trust is committed to safeguarding and promoting the welfare of children, young people and vulnerable adults and expects all staff and volunteers to share this commitment and promote safeguarding by implementing the Trusts policies and procedures, acting promptly on concerns, communicating effectively and sharing information appropriately. If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
