We have an exciting opportunity for 2 Mental Health Support Coordinators to play a vital role in providing support for our Emotional Wellbeing Service.
Position : 2x Mental Health Support Coordinator
Location : Hybrid roles with a mix of homeworking and within GP surgeries across Haywards Heath or Burgess Hill
Salary : £25,948
Hours : 37 hours
Contract: Permanent

About the Emotional Wellbeing Service:
This is an amazing opportunity to join an incredibly supportive team of people who provide mental health support to patients, directly within GP practices across Mid-Sussex and Crawley. The Emotional Wellbeing Service works with patients to create personalised support plans and deliver person-centred support using a range of grounding techniques, coping skills, CBT tools, goal setting and signposting. We also work in partnership with Sussex Partnership Foundation Trust and our clinical Mental Health Practitioners who work alongside the Mental Health Support Coordinators to provide clinical interventions for patients who may present with higher needs.
About the role:
We have two vacancies for this innovative role which will be based in GP surgeries in the Haywards Heath and Burgess Hill area. The exciting role of a Mental Health Support Coordinator is to effectively meet the needs of patients who are experiencing mental health challenges, through the provision of direct phone, video-call, and face to face support and by encouraging these patients to access and engage with community services. You will be joining a passionate team who work together to support individuals with mental health needs and empower each other to develop their own knowledge and skills. You will also have 1:1 support, group reflective practice and debriefs with a Mental Health Practitioner who provide clinical advice and guidance and support your own development of skills and knowledge of mental health issues and pathways.
You will be committed to delivering high quality preventative mental health interventions and will have experience of working with vulnerable people using person centred and empowering approaches. Excellent communication and organisational skills are essential as a Mental Health Support Coordinator along with energy and enthusiasm to support and empower patients to better manage their issues and create personalised support plans to achieve attainable goals.
You will be a part of the Adult Community Mental Health Transformations Teams initiative, "To increase understanding of mental health and wellbeing in our communities and make it easier and quicker for people to access the support they need, when they need it, close to home." as part of the NHS Long Term Plan.
