We are looking for a Senior Social Worker to join our Community Team in Horsham. If you are interested in working as part of a strong and dynamic social work service we would love to hear from you!
As a Social Worker in our Community Team your role will involve working with adults from 18 years and upwards with physical disabilities, acquired brain injuries, chronic health conditions, older people and people with dementia. Tasks will include assessments, reviews, safeguarding enquiries and mental capacity act assessments.
You will be required to undertake strengths-based assessments applying relevant legislation utilising social work skills and knowledge to develop and implement a plan of support, aimed at improving wellbeing and delivering outcomes for people as well as contributing to and developing skills in safeguarding.
You will be collaborating with people and family or friend carers to undertake strengths-based assessments and reviews, identifying eligible needs and personalised outcomes. You will be working within a supportive environment to apply your social work skills to develop and implement support plans, aimed at improving wellbeing and delivering outcomes, including utilising community assets, and ensuring that eligible needs are met.
You will be required to evidence expert knowledge through your Continuing Professional Development (CPD) and working towards a specialist post-qualifying award if applicable (e.g., Practice Educator, Approved Mental Health Professional or Best Interest Assessor). You will be able to provide supervision for unregistered and/or registered social care practitioners, including mentoring and coaching of students and apprentices.
