Social Care Assessor
Start date: ASAP
Contract length: Initially 3 months
Hours per week: 37 hours per week
Hourly rate: £18ltd per hour
Job Overview
Bournemouth Christchurch and Poole Council are seeking a Social Care Assessor to join their team. The successful candidate will be responsible for assessing the needs of vulnerable adults and ensuring the provision of care, advice, and support to meet their identified needs.
Key Responsibilities
- Assessing the needs of vulnerable adults
- Arranging provision of care, advice, support, and all other services required to meet identified needs
- Carrying out needs-led assessments
- Implementing, monitoring, and reviewing care plans and service provision
- Providing advice, support, and information to clients, their families, and carers, as well as other professionals, about all aspects of care provision for adults
- Liaising with other staff in Acute and Primary Health Care, Social Services and Provider Organisations to ensure that identified needs are being met in the most effective way possible
- Contributing to the Unit's objective of ensuring the provision of high-quality, cost-effective services to vulnerable adults
- Maintaining essential records of work done and providing information for statistical, financial, and planning purposes
- Undertaking such other duties as may be required from time to time commensurate with the level of the post
Requirements
- Recent work experience in adult social care or similar field, involving direct contact with members of the public and hospital environment
- Experience in working as part of a team liaising with health and social care professionals
- Awareness of social, legal, and political context within which social care services are delivered
- Ability to work independently and manage own workload
- Excellent communication and interpersonal skills
- Good IT skills and ability to input and access information on Unit computer databases
