Job description

Clinical responsibilities: Assess young peoples health and wellbeing needs both in relation to their hospital admission, and in relation to complex or chronic long term conditions or life limiting conditions. Plan and deliver transition programmes to address childrens complex and changing health and wellbeing needs To support the development, implementation, evaluation of care and the setting of standards of care for young people, including those with complex or chronic long term conditions or life limiting conditions in collaboration with the multidisciplinary team. Plan and deliver research/evidence based care and treatment in collaboration with other health professionals Liaise with other health professionals as necessary when risks and needs are beyond own competence and scope of practice. Work throughout the paediatric areas of the hospital such as Willow, Paediatric Outpatients, ED and Adult areas supporting the families of young people, including those with complex or chronic long term conditions or life limiting conditions. Develop own knowledge and expertise within the adolescent speciality, including complex, chronic long term conditions, life limiting conditions and the process of transition, relating to patient treatment, care pathways and health promotion. Monitor and review the effectiveness of interventions and care plans with patients and MDT agreeing changes to meet needs and established goals for young people.
