Job description

The post host holder will: Be responsible for delivering the specialist clinical pharmacy to Respiratory medicine. Deliver an embedded medical team pharmacy service, using specialist knowledge of medicines to provide advice to consultants, medical, nursing and pharmacy staff on all pharmaceutical aspects of medicines for patients. Optimise pharmacist practitioner and prescriber opportunities within Speciality Medicine and facilitate pharmacist prescribing initiatives. Deliver pharmaceutical care to patients within Speciality Medicine at AH; recommend, substantiate and communicate best therapeutic options for patients and implement changes in prescribing practice to ensure evidence-based medicine is followed as appropriate. To function as an independent prescriber within Respiratory medicine as agreed with clinicians and funding, complying with all requirements of the GPhC in relation to registration as an independent prescriber and the associated professional responsibilities. Identify medicine-related causes for admission and develop treatment plans to optimise care. Undertake risk management for the pharmacy service to allocated wards and ensure compliance with medicines legislation, the Trust Medicines Policy, and medicines related national and local guidelines. Undertake clinical audit of medicine use and pharmacy services within the Medical Division. Ensure pharmaceutical aspects of research and clinical trials in medicine comply with relevant legislation and conduct own practice-based audit/research on pharmaceutical aspects of acute medicine. Provide specialist pharmaceutical teaching to pharmacy, medical and nursing staff Introduce measures to ensure drug expenditure is within agreed limits across the Alexandra hospital as requested by the Director and Deputy Director of Pharmacy. This will include monitoring the use and funding of new medicines for the Area Prescribing Committee (APC) Function as an Independent Prescriber ensuring appropriate medicines are prescribed or de-prescribed after assessment. This will be in line with GPhC registration requirements and be formally agreed with all appropriate and relevant clinicians, as well as being within the departmental scope and funding for IP practice. As an IP pharmacist the post holder will: o Practice within the scope of GPhC registration o Adhere to the Medicines Policy in relation to scope and responsibilities of role o Comply with the Trust Medicines formulary o Be responsible for providing on-going evidence of competency in relation to their prescribing role Evaluate prescribing requirements, either making any necessary amendments to pre-admission medicines or in relation to the presenting complaint according to evidence-based practice, national/regional directives and clinical governance requirements Navigate/refer patient appropriately cross the healthcare economy to meet their specific care needs Manage and direct a ward-based medicines management pharmacy service to achieve Trust and departmental medicines management targets Be responsible for ensuring that the ward-based teams work efficiently, accurately and within their own individual job remits
