Remuneration: Circa £50,000 - £60,000

Description: Full time Performer Dentist required for 3-5 days (FLEXIBLE) in an INDEPENDENTLY OWNED, family run practice near Rugeley, Staffordshire. 
•    £13 PER UDA
•    50% on private and Labs
•    Existing, well maintained list
•    5 surgery practice
•    Provide Invisalign and Composite Bonding
•    Fully computerised with Clinipads and patient portal
•    iTero Scanner
•    Digital xrays
•    Rotary endo
•    Intra oral cameras
•    Qualified nurses
•    Parking on site
•    40 minutes to Birmingham and Stoke
•    Available now, start date negotiable

- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate

- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
