Remuneration: Circa £100,000+

Description: Part-time or full-time Performer Dentist required for 2-5 days per week (FLEXIBLE) in a busy, mixed, well established INDEPENDENTLY OWNED practice near Greengates, Bradford.
•    £13 per UDA
•    GOOD PRIVATE INCOME IN ADDITION - PRACTICE IN A NICE AREA
•    50% split on private and labs
•    Fully computerised surgeries - 3 surgery practice
•    Digital x-rays
•    Rotary Endodontics
•    Hygienist and Therapist support
•    Practice manager on-site
•    Existing, well maintained patient list
•    On-street parking
•    10 minutes to Bradford, 20 mins to Leeds, 30 minutes from Harrogate.
•    Available now, negotiable start date.

- Full NHS Performer Number if applicable
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years 
- Indemnity certificate

- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
