We have multiple new roles available to further expand the amazing Hawkes Bay clinical pharmacist facilitator team. This is your chance to join this team which is ‘long standing’ and continuously achieves great outcomes, as it continues to expand.
We have two vacancies available for Clinical Pharmacists based as part of the frailty team across Hawkes Bay. These roles will be integrated within the aged care facilities and primary care, supporting transition of care and appropriate prescribing of aged care residents.
The Hawkes Bay clinical pharmacist facilitator team is the largest within New Zealand and has a great support structure with its own team manager, two team leaders and a clinical educator. This close-knit autonomous team of Clinical Pharmacists offers a supportive environment in a role that will allow you to reach your potential to work at top of scope.
Your main responsibility is to provide direct clinical pharmacist facilitation services, focusing on the appropriate prescribing and utilisation of medicines to aged care facilities across Hawkes Bay.
