Role And Responsibilities

Consultation under the scope of expertise including:
Diagnosing and formulating suitable treatment plans for chronic care diseases.
Prescribing medications as required and providing pharmacological and nonpharmacological advice to the patients.
Prescribing diagnostic tests as required and discussing the results in follow-up consults if required.
Maintaining complete case reports including diagnosis, prescriptions, care plans, and patient progress in the Digital Platform
Escalation of patients requiring specialist intervention to the concerned Specialist.
Patient initial assessment and triaging of the severity of cases and providing care accordingly.
In severe cases, referral to hospitals and guidance and counseling to caregivers/ patients until a patient is admitted/ treated.
Follow-up calls to patients under the care plan at defined frequencies.
Need to perform virtual consultations for a continuum of care.
